I suspect that sometimes it's because the lower "lobe" of the headstock has been broken off.

Unfortunately I can't play mandolin; I was wondering about it as a gift for my bandmate - he's just retired (so I'd have an excuse :) ) and his existing mandolin is a nasty piece of tat which won't stay in tune (and has no pickup so has to be recorded with a microphone). As usual I'm trying to solve my problems by spending money on them
