Ta Les
I use these all the time at work, I coach people with Learning Disabilities so have to be covered in all areas. Make a table and use these as the column headings: Hazard, Outcome, People / property at Risk, Severity / Likelihood. Hazard: What is causing the risk? i.e. water, bad weather, messy wire on stage Outcome: What could happen? i.e. slips trips & falls. physical injury damage to instruments, death People / Property at Risk: Performers, audience, staff etc Severity / likelihood: Grade each one: A= Chance of Death B= Long term injury lasting 3 days or more, C= minor injury. 1=likely to occur, 2=chance of occurring 3=unlikely to occur. What do you need to do before the gig and what do staff need to do during the gig to continue doing during the gig? Just for your band playing, setting up and tearing down? Who is responsible for setting up the stage, providing services, security, welfare etc? What involvement do your local authorities have in the event? Wondering why you've been asked to complete one.

You may also need specific assessments for certain hazards, as well as a general risk assessment. The assessor may need expertise in some areas (electricity, work at heights, rigging, etc, in order to conduct a risk assessment and decide suitable precautions.
Cool. It just sounds a bit odd, probably a standard contract proforma or summat. The venue management are generally responsible for HSE compliance, your RA should only be for what your band are doing, it should be very simple. I'm surprised you've been asked at all. Les
Nige, all it says is "Please forward your insurance cover and risk assessment" It also says if outdoors then venue to provide adequate PA (ours won't cope) and if indoors band to provide PA. Also if we are prevented from performing due to inclement weather then the fee will still be payable. Thats it really. Les

Les
Ok, so you need a general risk assessment for your own activities - setting up, playing, tearing down etc. Is your kit PAT tested and do you use RCDs? Do you have public liability insurance? Your RA should be simple as possible. It's a simple five steps: ID the hazards; Decide who is at risk from your activities and how (you and the public); What are your existing precautions; what extra precautions need to be put in place; decide the likelihood and severity of of the risks, review and implement your assessment. You only need to consider significant risks that could cause real harm (electricity, manual handling, slips and trips, etc.) Don't get trapped into trying to prevent every possible eventuality. Have a quick look at this. http://www.hse.gov.uk/risk/casestudies/ I have to go out now but I'll be back about 6pm mate.
Les
Thanks for the help everyone. Gonna use one of the templates and knock one up. Sort of assuming that other than our equipment being safe and PAT tested everything other than our own house keeping on stage, like security to keep the public off, electrical supply etc is going to be he managements part. To be honest I think if I downloaded the NASA space walk risk assessment and sent it to the agent it would just be filed and not read. It's your responsibility now that the actions you have written on it are either carried out by yourself, or where you have noted, by others. If they're not then you don't play. End of. If the sh*t hits the fan, you can bet lots of very important people will read it.

Les
Yes, I am taking it seriously to be fair. I'm just not 100 per cent sure the venue is ever going to see it to be honest, I think it may just be the agent asking for it. Will nail it together and put it up here for an opinion. We're quite a long in the tooth easy going, will always find a solution band, but what happens if the venue haven't done any of the things you would expect ? Like no security to keep the public off the stage and out of harms way ? Can I just waive my risk assessment and say we won't be playing, give us our fee and we'll get out of your hair ?
TimR
It really depends on what their risk assessment says. Which you will obviously ask to see They will have decided what level of security to provide, probably it is specified in law or by their insurance. Although they've probably sub contracted that to a registered company too.
Les
Christ I'm tying myself in knots here, page seven hundred and sixty eight is dealing with a terrorist attack and the four minute warning of a nuclear strike Better get back to the basics
TimR
Lol. Just limit it to the type of things you know could happen. Not the things that might happen. If that makes sense. So electrically, someone could spill beer over cables or a drum stand spike could go through a cable. Either make sure gear is RCD protected or ban drinks and state no spikes on stands. People can trip over cables, route them away from exists and where you will be playing and gaffa tape lose ones that can't be rerouted. Don't block fire escapes etc. Just basic housekeeping that you would usually do but written down. Really doesn't need to be a 10 page document.
