Artec SE3P-A preamp, 3 band preamp with sweepable, semi-parametric mids, volume and blend controls. 4 pots in all, two of them dual concentric.

Hardly used, installed once, worked great with extremely low noise floor. Just gone back to passive. Comes with battery clip but no output jack.
