Pickup wiring help please

Hi, I have a question re pickup wiring as follows, I have a DS BS Curtis Novak pickup with the coil tap, I have got as far as having the pickup and a spdt switch and 0.47 orange drop and input jack, but I am drawing a blank on how to wire it up, the nasty drawing below is what I have got so far any help would be much appreciated.
