Whirlwind Bass 10 ten-band graphic EQ + Vol.  No box and has both velcro and little rubber feet on the underside.  

 

It's 18v but didn't come with any power adapter when I bought it, plus it lived on my pedalboard and drew its power from a powerbrick.  

 

These things get great reviews and for some reason hardly seem to come up for sale.
