This is a Chord CPT-01 that I had on my pedal board until recently. It has a lovely large LED display that can easily be seen on stage even with poor sight. The LEDs show up brighter in real life than in the pictures.

 

It can run on a power supply or internal 9V battery. There are two outputs, one mutes on tune. The other is on all the time. As it was on my pedal board, it has industrial strength Velcro on the bottom but it can be removed if you want.
