Can anyone identify this lead?

Bought the Planet Waved years ago and it fits a barrel Jack lovely but this is on the blink.  Would love to get a new one but where?

 

anyone know name of these with the extra connections pics below

 

can’t remember where I bought it so lifetime warranty gone. And D’addario seem to own PW now

Planet Waves has always been a D'Addario sub-brand. If it has a lifetime warranty its probably worth contacting them via the link at the bottom of https://www.daddario.com/

 

If it has failed at one end, I would snip off the dodgy connector an inch or so from the end & solder on a neutrik.
