Made to measure guitar neck plate - Any pointers or suggestions?

Hi,

 

I'm looking for a guitar neck plate for a 2nd hand guitar neck, specifically a Yamaha. Its quite a small plate and the gap between the screws doesn't fit any one I have. eBay has proved fruitless

 

Not looking to spend a fortune here, but does anyone have any suggestions for companies or people who can machine one please?
