Replaced in the original bass and now surplus to requirements:

 

 

So this is a 5 knobber with the following config:

 

Vol/pan/mids/bass and treble stack/passive tone

 

As per the picture two battery harness are available, 9 or 18V whichever you prefer. There is no jack socket though and no knobs. I can possibly rustle both up if you need some
