East UK U-Retro 5 Knob Deluxe (active/passive push-pull tone) with black knobs (only 3 springs for the single knobs, was fitted like this).

 

Fully working and tested (the back of the epoxy has been glued in the middle where it tends to break, but it's only cosmetical), comes with the original user manual.

 

For those who don't know it's the predecessor of the Uni-Pre.
