Generic 4 string Fender style bridge (chrome) with threaded saddles & flathead grub screws. Five screw mount.

 

I honestly can't remember where and when I bought this, but its never been used and its only ever been out of the packet to take the photos. Its also not a biological hazard, despite what the bag may say...
